A hospital group running the same PDCA cycle across sites in Zurich, Bern, Basel, Geneva and Lausanne had no shared record of it. An infection rate finding logged in Basel and a medication error logged in Bern sat in different files, so nobody could tell if a fix at one site ever reached another.

One system for the whole cycle: a finding opens in Plan, moves through Do and Check, and closes in Act with a documented outcome, standardize or improve and rerun. Every finding carries a location, a severity and an owner, and the activity log keeps a clinical audit trail of who moved what and when.

Virena now holds the group's patient safety record across every site instead of one folder per hospital. A standard that worked in Basel is visible in Bern before the same problem shows up there, and the audit trail behind any finding is one search away, not something someone has to remember.
